Letter from the President

According to a recent Mayo Clinic Study, over 50% of the physicians in the United States report being burned out. Honestly, that seems low to me. I’m sure that survey doesn’t include the doctors who don’t exhibit the signs of burnout, but are still very irritated with the current state of the practice of medicine. There are numerous reasons for this, and a short list would include:
  • Increasing administrative tasks (MOC, Meaningful Use, Patient Ratings, new EMR training, RVUs, etc.
  • Increasing work hours with diminishing returns
  • Increased expectations for immediate and real time communication (texting, patient portals and email)
  • Loss of autonomy
  • Computerization of medical care
  • Struggling to get care for patients (pre-certs, networks, formulary restriction, high deductibles)

Burnout is a symptom, not a disease. There has always has been physician burnout but it is increasing due to our current environment. There are many manifestations of it –

  • Physical and emotional exhaustion
  • Cynicism and resentment of the medical system and patients
  • Declining sense of accomplishment
  • Loss of joy

The above can lead to many negative behaviors or events. Drug and alcohol abuse, broken relationships, early retirement, abuse of staff members, general decline in professionalism and suicidal thoughts can all be connected to burnout. There are over 400 physician suicides a year in the U.S., and the suicide rate for doctors is 2-3 times the rate for the general population.  1,000,000 patients a year lose their doctors to suicide alone. Many more lose their doctors due to other ill effects of burnout. The effect on the family and friends of the burned out doctor go without saying.

What can be done? We are heading into an even more stressful time with an Epic conversion, MIPS, MACRA, an election that will likely have profound effects on our profession, and a payment system that is essentially broken. These challenges will stress every coping mechanism we have. The first line of defense is to take care of yourself. Spend MEANINGFUL time with family and friends. Eat healthy and exercise. Get better sleep. Say NO once in a while. You can’t solve all the world problems. We have to strike a balance between life and work. I know that’s easy to say, but it’s damn hard to do. Who has time for that? Geez, there is a new stress leading to burnout! I am expected to fix myself, and that is part of the problem.

If you are overwhelmed or you know someone who is, REACH OUT and talk. Sometimes just expressing yourself to another human is helpful. We, as a profession and a healthcare system, must be willing to tackle this issue. One of the best comments I’ve heard about physician burnout is that the only thing worse than having a physician suffer from burnout is to have a patient suffer because of it. Patient centered care doesn’t mean we can’t take care of ourselves. We must meet our own needs, so we can meet the needs of our patients.

INTEGRIS One Care powered by Epic: Provider Training Registration

On Sunday, October 30, 2016, INTEGRIS Baptist Medical Center, INTEGRIS Cancer Institute, and INTEGRIS Mental Health-Spencer will go-live on INTEGRIS One Care powered by Epic. All providers who work regularly at these facilities and expect to admit patients, make rounds, or perform a procedure on or after October 30, 2016 are required to complete training to work in Epic. Cerner will no longer be available at IBMC, ICI, or IMH-Spencer.

Training will take up to 20 hours in the classroom, depending on specialty. Please block your calendars accordingly to allow proper training time as this will affect your clinic and/or operating room schedules. Class sizes are limited. It is imperative to enroll early.


INTEGRIS-employed, in-patient providers not currently using Epic should contact their clinic or department managers to register.

INTEGRIS Medical Group providers and INTEGRIS Cardiovascular Physician providers currently using Epic Ambulatory should contact their clinic or department managers to register.

Independent providers with at least one year Epic experience within the previous 18 months on Epic versions 2014 or higher should call 405-633-9259 to determine training requirements.

All providers who do not fall within the groups above should call the INTEGRIS HealthLine at 405-951-2277, to register.

EpicCare Link

EpicCare Link is Epic’s web-based application for connecting organizations to community users. It provides community users secure access to select patient information in the INTEGRIS Epic data repository.

EpicCare Link is not an EMR solution. It is a read-only application with some service-oriented features that can be configured to facilitate communication with community practices, such as procedure order entry and appointment scheduling. Examples of community users include affiliated providers outside of INTEGRIS who need to review the clinical and administrative information of patients seen at INTEGRIS.

You and your staff can view a demonstration of EpicCare Link on Wednesday, October 12 at INTEGRIS Baptist Medical Center in the Auditorium at 3:00 p.m. OneCare staff will also be available to answer questions.

2015 Shared Savings Update

The 2015 claims data has been evaluated by Mercer Health and Benefits, LLC.,  a contracted vendor. IHP management is now analyzing the results in conjunction with INTEGRIS Health leadership in order to complete the shared savings plan. A tentative plan was presented to the IHP Board in August that will be finalized in the coming weeks. Then, the plan will be audited by our internal audit partner, Protiviti, to evaluate the findings and finalize individual shared savings.

The INTEGRIS Health Partners pharmacy committee met recently. They recommend the following exchanges:

Smoking is Not OK Campaign in October

INTEGRIS is sponsoring a statewide stop-smoking campaign during the month of October, Smoking Is NOT OK. Similar events in other states have been successful. The goal is to encourage Oklahomans to discuss quitting with their healthcare providers. To assist physicians with these discussions a new Web site may help you to help your patients quit smoking. www.advisethequit.com/carle includes video training to enable you to provide simple and effective smoking cessation counseling to patients. The video, which features a certified Tobacco Treatment Specialist, provides information on:

Why quitting can be challenging for patients: Knowing what their obstacles might be can help you to assist patients in overcoming them

How behavioral counseling can help: Treatment and support, combined, can give smokers improved chances of reaching their quit goals
