The healthcare landscape is changing rapidly. Every day there is a new merger, a new rule or a new realization of the true impact of the ACA on the delivery of healthcare in the U.S. Every conference I attend or article I read is about the changing environment of medicine in this country.

Patients are being asked to pay a higher percentage of the cost of their healthcare, and they have started to change their behavior as a result. They shop around for a lower cost alternative. There are many websites that tell a consumer the cost of a test or procedure at varying sites in their community. It is fascinating to watch how the behavior changes when the consumer has to pay for it.

Employers are in the same boat. They are looking for value in medical insurance benefits for their employees or considering dropping the benefit completely. The insurance industry is trying to provide products that are attractive and affordable. They are demanding quality that can be proven before they will even talk about reimbursement rates. This is true in the commercial market and in government funded healthcare.

There is only one conclusion I can draw from this: The healthcare world in this country has changed. There is a new “normal”. We will have to work together to survive it. There is great opportunity in the years ahead for those willing to work in a collaborative manner to deliver healthcare of high quality in an efficient way. It will require hard work and difficult decisions. There will be winners and losers. It is our job to be certain that the patient does not suffer and end up on the losing end.

We will have to understand the new “normal” to deal with it. ACO’s, Medicare Advantage, CIN’s, Patient Centered Medical Home (PCMH), Meaningful Use, etc. Part of INTEGRIS Health Partners' mission is to add value to your practice. We can do this in many ways. One way is to provide educational opportunities to our membership. We will soon be announcing speakers who will come to Oklahoma City and educate us on how to deal with all of these new challenges. One of these speakers is the Chief Medical officer of a MAJOR U.S. company. Another is the founder of the PCMH. Please take the time to attend these events when they happen.

The more we know about the current landscape, the better we will be able to deal with it successfully. IHP will provide us the vehicle to drive through this brave new world. The world is changing. Whether or not it is for better or for worse will depend on our response to these changes. Our patients are counting on us. Ultimately the Physician still directs healthcare. Understanding the current environment allows us to be part of the solution and an advocate for our patients.

2012 IHP Physician Satisfaction Survey Results
 
We want to thank the 130 IHP physicians for providing feedback by completing the 2012 Satisfaction Survey. This represented a return rate of 16% and the response from the IHP membership served as the foundation for the 2013 Strategic Initiatives recently approved by the IHP Board. Listed below is a summary of the key themes mentioned by the membership: 

  • 87% stated that they understood the goals of Clinical Integration
  • 73% stated they were very satisfied with the current leadership of IHP
  • 82% felt that they were well represented by the current governance structure
  • 74% felt that the newsletter was of moderate value
  • 62% felt that the IHP website was of moderate value
  • 64% felt that the in-office visits were valuable
  • Only 20% were currently using the MedVentive disease registry system
  • 56% of those using the system felt it was valuable
  • Only 50% had participated in the IHP educational offerings
  • 95% felt that IHP can be an effective vehicle in reducing inpatient hospital costs
  • 85% currently have an EHR in their offices
  • Over 75 disparate EHR systems were identified from the respondents

IHP has contracts with many laboratories, durable medical equipment providers, orthotics/prosthetics and LifeWatch monitoring services that are within our network. 2013 PCP Member Panels

On February 8, the IHP Care Coordination department sent letters to IHP Primary Care Physicians with their individual patient panels. Each PCP should have received a list of their IHP patients, plus whether or not any of their patients are enrolled in Care Coordination. This year there are 8,165 employees and covered dependents in IHP, up from last year’s membership of approximately 2,500.


Meet the Care Coordination Staff 

Tonia Garner is an RN with over 13 years of nursing experience in the areas of pediatrics, medical surgical, and intermediate care. She has worked in various settings, to include hospitals, clinics, and home care. Tonia spent four years as a case manager for INTEGRIS Health before joining IHP.



Susan Adams brings 37 years of nursing experience to the IHP Care Coordination team. She has a broad range of experience in inpatient and outpatient case management, program development, and nursing instruction. She earned an ADN from Westark Community College before completing a BSN at the University of Oklahoma. Prior to moving to Edmond, Susan lived in Tahlequah, where she worked for Indian Health Services and the Cherokee Nation.

Ann White began her career with INTEGRIS Health in 2002 in the Materials Management Department. A year later, she was blessed with an opportunity to serve the newest members of our community by working as a Health Unit Coordinator in the IBMC Newborn Nursery. While working for INTEGRIS, Ann earned a degree in Crime Victim Services, and has aided victims of domestic violence through research and work for a domestic violence shelter. After nine fulfilling years in IBMC’s Women’s Center, Ann has joined the Care Coordination team as Administrative Assistant.


The Patient Centered Medical Home

We have had much discussion within IHP regarding the development of the PCMH for IHP. Several doctors and clinics in IHP are currently participating in a Center for Medicare and Medicaid Innovation project with Transformed, VHA, and Phytel to help us develop a PCMH model, which is mentioned prominently in the Patient Protection and Affordable Care Act. Included in the standards are the following 10 requirements:

  • Written standards for patient access and patient communication
  • Use of data to show standards for patient access and communication are met
  • Use of paper or electronic charting tools to organize clinical information
  • Use of data to identify important diagnoses and conditions in practice
  • Adoption and implementation of evidence-based guidelines for three chronic or important conditions
  • Active support of patient self-management
  • Systematic tracking of tests and follow up on test results
  • Systematic tracking of critical referrals
  • Measurement of clinical and/or service performance
  • Performance reporting by physician or across the practice
