Referrals to the INTEGRIS hospice program may be initiated by a medical professional, community agent, friends, family member, or the patient. A nurse will consult with the patient, family and primary care physician to assess whether hospice care is appropriate and desired upon receiving a request for services.

- Amyotrophic Lateral Sclerosis (ALS)
- Cerebral Vascular Accident/Stroke or Coma
- Heart Disease/CHF
- HIV Disease
- Huntington’s disease
- Liver Disease
- Lung Disease/COPD
- Multiple Sclerosis
- Muscular Dystrophy
- Myasthenia Gravis
- Parkinson’s disease
- Renal Failure Chronic
Hospice care is covered under Medicare, most private insurance plans, HMO’s, and other managed care organizations.
