On Oct. 17, 2019, we learned that a portable hard drive containing patient information went missing during an office move that occurred shortly before that date. We immediately began an investigation and conducted a thorough search for the hard drive but were unable to locate it. We are currently analyzing the files known to be on the hard drive to identify the information it contained, including patient information. This analysis is ongoing, and we will send letters to patients whose information is contained on the hard drive once that investigation is complete.
We will also update this notice as we obtain more information about the contents of the hard drive.
At this time, we have no indication that any patient information has been accessed or misused. However, out of an abundance of caution, we have established a dedicated call center for patients to call with questions, at 1-833-991-1530, Monday through Friday, between 8:00 a.m. and 8:00 p.m. CST. We recommend our patients to review statements they receive from their health care providers. If they see services they did not receive, please contact the provider immediately. To help prevent something like this from happening in the future, we are performing additional education to ensure enforcement of our portable drive policy. We deeply regret any concern or inconvenience this incident may cause our patients.
