At your appointment, you can expect the following: a review of your medical and family history; develop or update of a list of current providers and prescriptions, height, weight, blood pressure, and other routine measurements; detection of any cognitive impairment, personalized health advice, a list of risk factors and treatment options for you and a screening schedule for appropriate preventive services.
A wellness visit doesn't address new or existing health problems. Please let the scheduling staff know if you need the provider's help with a condition outside of wellness, a medication refill or anything else.
If your provider completing your INTEGRIS Health Wellness Visit is NOT on Epic, or your provider is outside the INTEGRIS Health Partners Network, please print the form below.
You can complete your INTEGRIS Health Wellness Visit virtually. This option is for existing patients only. New patients must complete their visit in the office.

In order to help you qualify for the cash incentive, lab work MUST be completed as part of the INTEGRIS Health Wellness Visit and include a lipid panel, Basic Metabolic Panel (BMP), and Hemoglobin A1c (HbA1c). NOTE: if you have previous results, a second lab draw will be covered under the INTEGRIS Health Medical Plan if the IH Modifier is attached to the visit.
