2020 Wellness Cash Incentive

In 2020, caregivers and covered spouses, on the INTEGRIS Medical Plan, each have the opportunity to earn up to a $400 Cash Incentive. To earn the $400 Cash Incentive, select from the menu of options below to help you earn the maximum incentive. Only one $100 program option will count toward the 2020 maximum incentive of $400. 

$300 - 2020 INTEGRIS WELLNESS VISIT AND CURRENT WITH PREVENTIVE CARE GUIDELINES

$100 - LIFESTYLE MANAGEMENT AND MAINTENANCE PROGRAMS

$300 - MATERNAL WELLNESS PROGRAM

$100 - NATIONAL DIABETES PREVENTION PROGRAM

$100 - CARE COORDINATION PROGRAM

The incentives apply to all caregivers and spouses enrolled in the INTEGRIS Medical Plan.  To be eligible, you must be employed at the time of payout as well as enrolled in the medical plan as of October 1, 2020


Program Deadline

All program requirements and verification forms must be met and submitted to Employee Wellness by October 31, 2020


Dashboard

Please be sure to check your Employee Wellness Dashboard frequently. Your dashboard reflects the information that Employee Wellness has received. If you dashboard is not reflecting correctly, it is your responsibility to notify Employee Wellness to rectify the situation. Incomplete dashboards will result in the caregivers/covered spouses not receiving the full 2020 cash incentive amount.


Wellness Program Privacy

Your participation in the Wellness Program is completely voluntary. You have the option to stop participating in the Wellness Program at any time. Any personal medical information that you share in connection with the Wellness Program will be protected and confidential and may not be used for employment purposes.
