2022 Wellness Cash Incentive

In 2022, caregivers and covered spouses, on the INTEGRIS Health Medical Plan, each have the opportunity to earn up to a $400 Cash Incentive. To earn the $400 Cash Incentive, select from the below options to help you earn the maximum incentive.

$200 - 2022 INTEGRIS WELLNESS VISIT OR MATERNAL WELLNESS PHYSICIAN VISIT 

$200 -CURRENT WITH PREVENTIVE CARE GUIDELINES 

 

The incentives apply to all caregivers and spouses enrolled in the INTEGRIS Health Medical Plan.  To be eligible, you must be employed at the time of payout as well as enrolled in the medical plan as of October 1, 2022


Program Requirements

All program requirements and verification forms must be met and submitted to Caregiver Wellness by October 31, 2022.

Uploads or emails are not necessary for caregivers who have an INTEGRIS provider. For caregivers who have providers outside of the INTEGRIS system, have your provider fill out the “Provider Verification Form” and email it to EmployeeWellness@integrisok.com


READYSET - THE NEW SAFE AND SECURE CAREGIVER HEALTH DATABASE PROGRAM

ReadySet is Caregiver Health's new safe and secure employee health program. The Dashboard is no longer available. To access your ReadySet account, follow this single sign on link

HOW TO SUBMIT:

• Caregivers who have an INTEGRIS health care provider do not need to upload any information into ReadySet. 
• For caregivers who are participating in the incentive program but have a healthcare provider outside of the INTEGRIS system, complete the Wellness Incentive “Provider Verification Form”  (link to the form) and email it to EmployeeWellness@integrisok.com
• Caregiver Health and Wellness team members will securely review and determine whether all requirements have been met by any caregiver wishes to participate. 
Once reviewed, approval will appear in the caregiver’s ReadySet account as “reviewed” in the documents section.  

IMPORTANT DATES:

• All wellness incentive visits, and documentation must be completed or emailed by October 31st, 2022, with no exceptions. Documentation received after October 31st will not be included in the 2022 Wellness Incentive program.  
• Caregiver Health and Wellness will be reviewing all wellness incentive documentation throughout the months of October and November. 
• Caregivers may view confirmation documentation within their ReadySet personal portal under "documents".

 

Wellness Program Privacy

Your participation in the Wellness Program is completely voluntary. You have the option to stop participating in the Wellness Program at any time. Any personal medical information that you share in connection with the Wellness Program will be protected and confidential and may not be used for employment purposes.
