2019 Wellness Cash Incentive

In 2019, caregivers and covered spouses, on the INTEGRIS Medical Plan, each have the opportunity to earn up to a $400 Cash Incentive. To earn the $400 Cash Incentive, select from the menu of options below to help you earn the maximum incentive. You can complete more than one Lifestyle Management and Maintenance program to help you earn the maximum incentive.

$200 - LIFESTYLE MANAGEMENT AND MAINTENANCE PROGRAMS

$200 - MATERNAL WELLNESS PROGRAM

$200 - NATIONAL DIABETES PREVENTION PROGRAM

$200 - CARE COORDINATION PROGRAM

$20-$100 Pop-Up Programs

The incentives apply to all caregivers and spouses enrolled in the INTEGRIS Medical Plan.  To be eligible, you must be enrolled in the medical plan as of October 1, 2019


Wellness Program Privacy

Your participation in the Wellness Program is completely voluntary. You have the option to stop participating in the Wellness Program at any time. Any personal medical information that you share in connection with the Wellness Program will be protected and confidential and may not be used for employment purposes.
