INTEGRIS Wellness Visit and Preventive Care

Complete your 2020 INTEGRIS Wellness Visit (including labs) between January 1st and October 31, 2020. Also, age and gender appropriate preventive care must be current. Preventive care visit may not be required this calendar year if you are current on the below guideline. Please note, the frequency of the preventive care may be different from what is listed below, for you. Please discuss your personal care plan with your physician.

Please note: you MUST wait to complete your INTEGRIS Wellness Visit and/or preventive care after your INTEGRIS Medical Plan effective date.

Maternal Wellness


INTEGRIS Wellness Visit 

At your appointment you can expect the following: a review of your medical and family history, develop or update a list of current providers and prescriptions, height, weight, blood pressure, and other routine measurements, detection of any cognitive impairment, personalized health advice, a list of risk factors and treatment options for you, and a screening schedule for appropriate preventive services.

A wellness visit doesn't address new or existing health problems. Please let the scheduling staff know if you need the provider's help with a condition outside of wellness, a medication refill or anything else.

If your provider completing your INTEGRIS Wellness Visit is NOT on Epic, or your provider is outside the INTEGRIS Health Partners Network, please print the form below.

Wellness Labs

In order to help you qualify for the cash incentive, lab work MUST be completed as part of the INTEGRIS Wellness Visit and include a lipid panel, Basic Metabolic Panel (BMP), and Hemoglobin A1c (HbA1c). NOTE: if you have previous results, a second lab draw will be covered under the INTEGRIS Medical Plan if the IH Modifier is attached to the visit.  


Preventive Care Guidelines

Cervical Cancer Screening (pap/pelvic exam)

  • Age 21-64; every 3 years
  • Age 30-64; every 5 year if HPV testing performed

Breast Cancer Screening (mammogram)

  • Age 40-49; talk to your primary care provider about when to start and how often
  • Age 50-74; every 2 years

Colorectal Cancer Screening (current on one of the below)

  • Colonoscopy: Age 50-75; every 10 years
  • Fecal occult blood test: Age 50-75; annually
  • Fecal immunochemical test (FIT): Age 50-75; annually
