Letter from the President

Dear colleagues,

The beginning of this new year finds us in the middle of a dramatic discussion about opioid use and its effects on our society. It is fascinating to watch the process evolve and the rapid efforts being made to stem the tide of opioid addiction. The medical community is being asked to help solve this problem.

We can expect many new guidelines, recommendations and rules to be rapidly deployed. INTEGRIS Health is engaged and is formulating a system wide approach. This will include ambulatory and inpatient strategies and resources.

As of January 1, 2018, the Joint Commission has issued new Standards Revisions Related to Pain Assessment and Management intended to measure an organization’s role in addressing the opioid epidemic from the perspectives of leadership, medical staff, and provisions of care/treatment and services. An interdisciplinary team with members from across the system has been assessing and developing standardized shared work products to meet these new guidelines. System-wide communication and education is being developed for physicians and clinicians to utilize best practices identified throughout the state and country.

INTEGRIS has always been a leader in the efforts to improve the health and wellness of our communities. The development of Arcadia Trails for the treatment of addiction is a perfect example of the INTEGRIS commitment. Please stay aware of information we will share regarding the opioid issue. We will provide information and insight that is meaningful and appropriate for our staff, patients and providers. Several resources are included in this newsletter as a starting point.

A well thought out and cohesive response can improve the current situation. We all need to work together to get this done. We are planning a role out of our comprehensive response later in the year. Stay tuned.

Centers for Disease Control Guidelines for Prescribing Opioids for Chronic Pain

This guideline provides recommendations for primary care clinicians who are prescribing opioids for chronic pain outside of active cancer treatment, palliative care, and end-of-life care. The guideline addresses 1.) When to initiate or continue opioids for chronic pain; 2.) Opioid selection, dosage, duration, follow-up, and discontinuation; and 3.) Assessing risk and addressing harms of opioid use. CDC developed the guideline using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) framework, and recommendations are made on the basis of a systematic review of the scientific evidence while considering benefits and harms, values and preferences, and resource allocation. CDC obtained input from experts, stakeholders, the public, peer reviewers, and a federally chartered advisory committee. It is important that patients receive appropriate pain treatment with careful consideration of the benefits and risks of treatment options. This guideline is intended to improve communication between clinicians and patients about the risks and benefits of opioid therapy for chronic pain, improve the safety and effectiveness of pain treatment, and reduce the risks associated with long-term opioid therapy, including opioid use disorder, overdose, and death.

CDC has provided a printable checklist for prescribing opioids for chronic pain (http://stacks.cdc.gov/view/cdc/38025) as well as a website, (https://www.cdc.gov/drugoverdose/prescribing/resources.html) with additional tools to guide clinicians in implementing the recommendations.

Metro physicians: The Metro Summits are scheduled for Thursday, March 29, 5:30 p.m. (see above for registration information), Tuesday, June 5, 5:30 p.m., and Tuesday, October 2, 5:30 p.m. Registration for the June and October summits will be available in the month prior to each.

Attending in person or viewing a summit via livestream is a required operational metric for all specialties in order to participate in shared savings.
