Letter from the President

Dear colleagues,

Bruce Lawrence’s vision of a clinically integrated network and coordinated care delivery led to the formation of INTEGRIS Health Partners. Many people participated in the development of IHP; however, Bruce’s leadership and vision were instrumental to the successful launch. IHP was the first clinically integrated network in the state and continues to be a unique organization for our area.

Bruce’s recent announcement of his retirement as president and chief executive officer of INTEGRIS is a bittersweet event for me. I know we all wish him the best and are probably jealous that he will have shed the stress of working in the healthcare industry in these unsettled times. It is however, worrisome to lose the person who had the foresight to set us on the path to clinical integration.

I want to assure everyone that the support from INTEGRIS is unwavering. Pete Delaney, current Board Chair for INTEGRIS, recently told me, “The INTEGRIS Board fully supports the vision and mission of IHP now and for the foreseeable future. IHP is an important and integral part of INTEGRIS.“ Several other board members have echoed that same sentiment to me, as well.

It seems that the only thing that we can count on is change. Many people who were instrumental in the development and success of IHP have moved on or retired. We haven’t missed a step through any of these transitions. We have the support we need to continue our important work and to move IHP forward.

We appreciate the incredible opportunity Bruce’s vision for IHP gave us. Let us be certain to continue to make IHP even stronger and serve as a testament to his legacy at INTEGRIS.

Coding & Documentation Improvement Training: Metric for Metro Hospital-based Medical and Surgical Physicians and Office-based Specialists

Completing Clinical Documentation Improvement training is a 2017 metric for metro physicians who fall into one of the following categories: 1. hospital-based medical and surgical or 2. clinic-based specialist. If you are not a primary care physician, please view the CDI video to fulfill the CDI training metric. PCPs are welcome to view the video, but it is not a required metric for PCPs.

Attending or viewing an INTEGRIS Health Partners summit is a required metric to participate in 2017 shared savings.

Metro physicians: View the final summit here if you did not attend a summit in March, June, or October.

Enid physicians: View the Enid summit here if you did not attend in November.

In 2017 all IHP education opportunities are online – on your time, at your convenience. Three videos are available now; only one is required to meet the educational metric for metro physicians.

2017 Diabetic Metric: Medical Attention for Nephropathy

Requiring diabetes patents age 18-75 to have a nephropathy assessment is a clinical metric for primary care in 2017. Clarification for meeting the metric is provided below.

Urine test for albumin or protein (any of the following meet criteria for a nephropathy screening or monitoring test)

a. 24 – hour urine for albumin or protein

b. Timed urine for albumin or protein

c. Spot urine (e.g. urine dipstick or test strip) for albumin or protein

d. Urine for albumin/creatinine ration

e. 24 – hour urine for total protein

f. Random urine for protein/creatinine ratio

CPT codes that would satisfy the above requirements are 82042, 82043, 82044, 84156, 81000, 81001, 81002, 81003, 81005

In addition to the lab tests, other ways to meet this metric are listed below:

· Documentation of a visit to a nephrologist.

· Documentation of a renal transplant.

· Documentation of medical attention for any of the following (no restriction on provider type)

o Diabetic nephropathy

o ESRD

o Chronic renal failure (CRF)

o Chronic kidney disease (CKD)

o Renal insufficiency

o Proteinuria

o Albuminuria

o Renal dysfunction

o Acute renal failure (ARF)

o Dialysis, hemodialysis or peritoneal dialysis

· Evidence of ACE inhibitor/ARB therapy. Documentation in the medical record must include, at minimum, a note indicating that the member received an ambulatory prescription for ACE inhibitors/ARBs in the measurement year.

Comprehensive Medication Management: What it is and how to order it

The INTEGRIS Health Partners Care Coordination program has added clinical pharmacists to offer Comprehensive Medication Management. The CMM program can provide a single appointment for a complete medication review and longitudinal care in collaboration with the physician to assist with asthma, diabetes, hypertension, or hyperlipidemia management and education. This appointment is FREE OF CHARGE to INTEGRIS employees and their dependents on the medical plan. The initial CMM appointment includes:

  • Medication history and reconciliation
  • Evaluation of the drug regimen for adherence, current indication, drug interactions, adverse drug effects, effectiveness and affordability
  • Medication education emphasizing purpose, proper administration and common adverse effects
  • Recommendations for optimizing medication regimens made to the prescriber and implemented with the prescriber’s approval

Patients referred for longitudinal CMM visits receive chronic disease management for asthma, diabetes, hypertension and hyperlipidemia in accordance with physician-approved protocols. The clinical pharmacists providing CMM services are Susan Conway, Pharm.D., BCPS, Kendall Powers, Pharm.D., BCACP, and Erica Smith, Pharm.D., BCACP. Physicians can refer patients to the CMM service through Epic using the “ambulatory referral to comprehensive medication management,” which is REF20002, or by contacting the clinical pharmacists at 405-951-2377. Patients must be INTEGRIS employees or their dependents on the INTEGRIS medical plan to be referred to CMM.


Pharmacy Update

The IHP Pharmacy Committee met recently. The committee makes specific medication recommendations for lice treatments and Minastrin24, attached.
