  • CMM Hours and Locations
    • 8:30 a.m. - 12 p.m. and 1 - 5 p.m. Mondays through Fridays (hours vary by location)
    • Appointments offered at multiple locations: 
      • 3435 NW 56th St Suite 101 at INTEGRIS Baptist Medical Center
      • 4221 S. Western Suite 1045 at INTEGRIS Southwest Medical Center
      • INTEGRIS Medical Group locations with embedded pharmacist
  • Patient Eligibility: 
    • Must be an INTEGRIS employee or their dependent AND enrolled in the INTEGRIS Medical Plan
    • A CMM visit can be scheduled based on a direct referral from the physician, care coordinator, or patient. Referral for an initial CMM visit can be made by calling 405-951-2377 or sending a referral through Epic. 
  • Services Offered 
    • Initial comprehensive medication management (CMM) visits include a comprehensive medication history and review; assessment of safety and efficacy of drug therapy, drug interactions, compliance, and costs; and medication education.
    • Follow-up CMM visits are scheduled based on a physician referral order to assist in the longitudinal management of asthma, diabetes, hyperlipidemia, and hypertension.
    • The clinical pharmacists provide CMM services in collaboration with the patient’s care coordinator, PCP, and other providers.
