The INTEGRIS Health Plan relies exclusively on a provider network known as the INTEGRIS Health Partners (IHP), to provide primary care and specialty services to its members. Requests for out-of-network services will only be considered for services that are not currently available within the IHP network.
In order to assist you and your health provider in supplying the information needed to process an exception request, the following form is provided. Please note any approval for exceptions must be granted before services are provided to be eligible for reimbursement by the plan. As the network is subject to change, you will need to call 405-951-2504 and request a new out-of-network authorization each year.
