New Hire Paperwork


Thank you for your continued cooperation during this exciting transition. As we are approaching the transition effective date, it is time to start completing new hire paperwork. In an effort to streamline this process, you will be using a system called “Signup Genius” to schedule your individual appointment time to complete your new hire paperwork. This should help alleviate wait times and allow for one-on-one assistance.

INTEGRIS Human Resources will be onsite in the cafeteria conference room beginning Wednesday, September 5th thru the 21st to assist you in completion of your paperwork. When you use the Signup Genius system, it will be important for you to schedule a time that does not conflict with your work schedule. If that isn’t possible, please work with your manager to determine the best time for you to schedule your appointment, so that it will not disrupt operations or patient care.

Paperwork should take no longer than 30 minutes to complete, provided you have finished your onboarding documents through the Taleo Employment portal. If you have not completed these documents online, we can assist you during your paperwork appointment. You will need to bring your bank direct deposit account information to your appointment.

As part of your new hire paperwork, you will need to complete an I-9 form which requires you to bring two specific forms of identification. You are required to bring documents that establish both identity and employment authorization. ID’s must be current (not expired) and your name should match the Social Security Office’s record. Please review the attached “I-9 List of Acceptable Documents” and have a suitable combination of documents with you for your paperwork appointment. You must have these documents with you to complete your I-9 form or your new hire start date may be delayed.

In addition to your I-9 identification, please bring copies of any licensure and/or certification (BLS, ACLS, PALS, NRP) cards required for your job.

HOSPITAL EMPLOYEES ONLY will need to sign up through the link below to schedule a new hire paperwork appointment. Clinic employees’ new hire paperwork will be coordinated through your clinic leadership – please DO NOT schedule an appointment time. Also, if you are a current INTEGRIS employee, please DO NOT schedule an appointment. Your records will be updated automatically to reflect your Deaconess position.

The link below will take you to the Signup Genius website. Once you are on this page, you will see tabs showing the available dates. Each date tab will list the available time slots throughout the day. The time slot will indicate how many openings are available or if that time is full.

To sign up for an opening:

  1. Choose a tab for an upcoming date in September. If you do not see a red box stating “Sign up”, all the slots are filled. You will need to select another date.
  2. Click the red box to insert a check mark next to the words “sign up” for the time slot you would like to select. Next click “submit and sign up” at the bottom of the page.
  3. This will open a page displaying the appointment time you have chosen.
  4. Add your first and last name and email address, then choose “sign up now”.
  5. You will see a confirmation on the screen as well as receive an email listing your appointment time.

*We have tried to provide a variety of times to accommodate your schedules; however, if you have extenuating circumstances, please let us know and we will work to meet with you at a different time.
