At INTEGRIS Caldwell Family Clinic, we understand that nothing is more important than the health of you and your family. That's why we offer a full range of primary care services for people of all ages - from infants and children to adults and seniors.

Our providers offer complete care for families, including primary care clinic services such as annual exams, immunizations, and well-child checks. Patient Information

We hope that we are doing everything possible to make you feel comfortable in our clinic. We strive to create an environment that is caring, safe, clean and cheerful. We appreciate you recommending us to your family and friends. We would love to make them part of the INTEGRIS Family Care system.

So we can ensure your information and medications are up to date, please bring the following with you when you come for your appointments.

  • List of health care providers you have seen since your last visit
  • Medications (prescriptions, over the counter medications, herbal medications and vitamins)
  • Your copay or deductible (if any) as indicated by your insurance plan coverage
  • Photo ID
  • Insurance card

Please remember, anytime you are admitted to the hospital or treated in the emergency room, please contact your primary care provider immediately.

Most providers see patients from 8 a.m. to 5 p.m. Some providers see patients before or after hours. Please contact your INTEGRIS office directly for other available hours.

Please contact our INTEGRIS practice directly if you feel you have an urgent situation. Your provider may direct you to an INTEGRIS Urgent Care facility or Access Medical Urgent Care. You may call 1-855-603-1147 to locate a center near you.

It is important we have the correct information to contact you and to file your insurance, we do this as a courtesy to you. Our goal is to be as accurate and efficient as possible.

Please don’t hesitate to ask questions should you have concerns about your medications or care. We’re here for you! We will provide you with a summary of your visit. This information will also be available on INTEGRIS & Me, our helpful patient portal.

If your lab results require a change in treatment, you will be contacted within two business days. If your results are normal, or require only a medication adjustment, you will receive your results within 10 business days. Referrals will be done within 10 business days unless it is an emergency.

Please contact your pharmacy directly for your medication prescription refills. The pharmacy will contact us if additional information is needed.

We will work to return phone calls as timely as possible. However, calls received in the afternoon may be returned the next business day.
