Interdisciplinary Team Approach

The INTEGRIS Hospice program uses the interdisciplinary team concept and our focus is on the whole person - the mind, body and the spirit. Our goal is to make every day a celebration of life.

Our Hospice team is highly trained, compassionate individuals who provide quality care for patients of all ages. Each team consists of a registered nurse, a social worker, home health aides, chaplain and a volunteer. The RN’s are specifically trained in pain and symptom management, and they act as the case manager. The RN visits patients weekly or as often as necessary to insure maximum comfort. A nurse is on call 24 hours a day, 7 days per week and our certified home health aides are assigned to attend to the personal care needs of the patient. Our social workers provide a variety of supportive services, which may include counseling, financial resource needs and assistance with funeral planning. Our chaplains are available to assist our patients and families with spiritual counseling as well as perform funeral services. Our volunteers companionship, run errands, provide light housekeeping, meal preparation, and shopping.

It’s important to note that the patients’ physician is an integral part of our team. The interdisciplinary team maintains regular contact with the primary care doctor. The INTEGRIS Hospice medical director’s meet weekly with the hospice team to discuss patient care. This information can also be shared with the patient’s doctor.
