Every injury and individual are different. There is no cookie-cutter approach, as each individual will respond differently to things. In addition to three hours of physical, occupational and/or speech therapy five out of seven days, you may have:

  1. Bowel/Bladder Programs
  2. A specialized room

What follow-up or life long care is available?‬‬

Discharge can be an overwhelming and challenging experience. The patient and family are returning home without a call light to gain immediate assistance. Making the transition home as easy as possible, is priority. The following are options to help ease the transition:

  • Coordination of Care – Communication is key. Through a tour and verbal communication, information is passed from one setting to the next through transition of care. Some of the information passed on include: current level, goals, progress made, challenges, family support, etc. This makes the transition easier and seamless.
  • Follow-up Clinic – Dr. Self, INTEGRIS Jim Thorpe Spinal Cord Injury Medical Director, provides a follow up clinic in which she will follow an individual's care 1 month, 3 months, 6 months, and 12 months after discharge. She will address SCI related needs and make referrals to other programs and physicians as needed. Many individuals with SCI are readmitted to the hospital within the first year of injury due to SCI related complications. Attending the follow up clinic at regularly scheduled intervals will help decrease the likelihood of re-hospitalization. The social worker will set-up this appointment with Dr. Self prior to discharge.
  • Survivor Group – INTEGRIS Jim Thorpe has monthly survivor groups that provide camaraderie and education. It meets the first Thursday of every month, from 5:30 to 7pm.
  • Continued Therapy – INTEGRIS Jim Thorpe Rehabilitation provides follow-up care through outpatient or home health. The south outpatient facility, located at 4100 S. Douglas, Oklahoma City, Oklahoma, 73109 specializes in SCI recovery.
