We have many various appointments to suit your needs. See below for details on each appointment type to help guide you. If you’re unsure of what to schedule, call our office and speak with one of our caregivers to ensure the appropriate appointment is scheduled.

You’ve seen this provider within the past two years and you have something you need to discuss. This appointment type usually addresses up to three issues. Extra time or another appointment may be needed to discuss more issues or concerns.

At your appointment you can expect the following.

  • Review of your medical and family history
  • Develop or update a list of current providers and prescriptions
  • Discussion of developed issue or concern
  • Develop a care plan

A yearly check-up to discuss changes, plan wellness improvements and build your relationship with your provider.

At your appointment you can expect the following.

  • Review of your medical and family history
  • Develop or update a list of current providers and prescriptions
  • Height, weight, blood pressure and other routine measurements
  • Detection of any cognitive impairment
  • Health maintenance and recommended immunizations
  • Personalized health advice
  • A list of risk factors and treatment options for you
  • A screening schedule (a checklist) for appropriate preventive services

A wellness or annual visit doesn’t address new or existing health problems. Please let our scheduling staff know if you need the provider’s help with a condition outside of wellness, a medication refill or anything else. Because it will take your provider additional time to address all your concerns, we’ll be happy to schedule another appointment for you. We want you to be happy with your care. Depending on your insurance coverage, a separate out-of-pocket cost might be due, whether provided on the same day or one different from your wellness visit.

What is Covered by Your Insurance

If you need to schedule a “Wellness or Annual Visit,” please make sure you let us know at the time of scheduling your appointment. Insurance companies require specific information for these visits and will cover only certain items; therefore, if you have other needs, you may need to schedule an additional appointment to address those concerns. If both concerns are addressed during the “Wellness Visit,” then it cannot be billed as a “wellness visit” and the Insurance will not pay as a “wellness visit.” Also, please note that not all labs are paid under a wellness code. Please check with your insurance to verify what services are covered under a wellness visit.

We’ll get you in fast for that issue that just popped up. Select this for an acute office visit to address a cold, rash or fever. This visit is to address one immediate issue.

  • Prescription, if necessary

A yearly check-up for females to discuss changes, plan wellness improvements and build your relationship with your provider.

At your appointment you can expect the following.

  • Height, weight, blood pressure and other routine measurements
  • Pelvic exam
  • Breast exam
  • Recommended immunizations
  • Birth control discussion (if applicable)
  • Sexual health discussion (if applicable)
  • Hormone imbalance discussion (if applicable)

At your appointment you can expect the following.

  • Discuss any issues with school or social groups
  • Any necessary vaccinations for that age

Save time and energy by having an E-Visit right now. Select this for minor acute issues like coughs, UTIs or Red/Pink Eye.

An E-Visit is a great tool for patients who want to handle an issue quickly, but without having to come into the office. This service is for established patients only. All you have to do is log into your INTEGRIS & Me account and select “E-Visit” for your appointment type. From there, you’ll select your issue and fill out a simple questionnaire. Your message will be sent to your provider and you’ll receive a reply within 24-48 hours.

Following are common issues that can be handled with an E-Visit.

  • Cough
  • Flu
  • Urinary tract infections
  • Red/Pink eye
  • Diarrhea

The best way to prevent the flu is to get an annual flu shot. Use this to arm yourself this flu season.
