A yearly check-up for INTEGRIS Health employees and covered spouses to discuss changes, plan wellness improvements and build your relationship with your provider.
At your appointment you can expect the following.
- Review of your medical and family history
- Develop or update a list of current providers and prescriptions
- Height, weight, blood pressure and other routine measurements
- Detection of any cognitive impairment
- Personalized health advice
- A list of risk factors and treatment options for you
- A screening schedule (a checklist) for appropriate preventive services
A wellness or annual visit doesn’t address new or existing health problems. Please let our scheduling staff know if you need the provider’s help with a condition outside of wellness, a medication refill or anything else. Because it will take your provider additional time to address all your concerns, we’ll be happy to schedule another appointment for you. We want you to be happy with your care. Depending on your insurance coverage, a separate out-of-pocket cost might be due, whether provided on the same day or one different from your wellness visit.
What is Covered by Your Insurance
If you need to schedule a “Wellness or Annual Visit,” please make sure you let us know at the time of scheduling your appointment. Insurance companies require specific information for these visits and will cover only certain items; therefore, if you have other needs, you may need to schedule an additional appointment to address those concerns. If both concerns are addressed during the “Wellness Visit,” then it cannot be billed as a “wellness visit” and the Insurance will not pay as a “wellness visit.” Also, please note that not all labs are paid under a wellness code. Please check with your insurance to verify what services are covered under a wellness visit.
