When choosing a health care proxy, ask those you are considering if they are willing to:

  2. Talk with you about your health care goals, values and preferences
  3. Follow your decisions, even if they do not agree with them
  4. Make decisions in difficult situations.

After you appoint your health care proxy, it is crucial to discuss your health care goals with them. Also provide them a copy of your advance directive.
