  • Life Insurance policy up to the lesser of $6,500,000 or 7x annual base pay
  • Personal Accident policy up to the lesser of $1,500,000 or 7x annual base pay
  • Short Term Disability 60% income replacement coverage (up to 2,300 per week)
  • Long Term Disability 60% income replacement coverage (up to 15,000 per month)


  • Annual INTEGRIS Contribution equal to 3%-6% of annual compensation (annual considered compensation is limited to $265,000 in 2016) Contribution percentage is determined by tenure 0-4 years = 3% | 5-14 years = 4% | 15-19 years = 5% | 20+ years=6%
  • Executive Supplemental Retirement Plan or Non-Qualified Deferred Compensation Plan
    Physician can elect to voluntarily defer compensation ($18,000 maximum in 2016) through a non-qualified plan Participation can be concurrent with Retirement Savings Plan participation There is no vesting requirement to either the ESRP or NQDC plans, however contributions are not “owned” by participants until distribution
